Contact our Louisville office
5813 Bardstown Rd Unit 204
Louisville, KY 40291
Call 502-234-1515 or use the care inquiry form. If you would like to meet in person, contact us to discuss arrangements before coming to the office.
The Louisville address is an office contact point, not an adult day healthcare location. Better Life’s adult day healthcare program is available only in Frankfort.
Begin with the routine at home
A Louisville family might be looking for help with getting dressed, preparing meals, or keeping laundry from becoming an unfinished task. Another person may need support during a part of the day when their family is at work. Describe the activities and the times that matter, so the conversation can focus on a workable arrangement.
Mention the ZIP code where care is needed, even if it differs from the mailing address or the place where a family member lives. Home care is delivered at the person’s home; the office address does not define the service boundary.
- Which personal-care activities need assistance?
- What household tasks relate to the person’s daily routine?
- Who already helps, and which parts of the week need more support?
- What does the person want to continue doing independently?
Discuss the payment route early
Better Life offers private-pay home care and services through the HCB Waiver for eligible participants with authorized care. If you are paying directly, ask about the proposed scope, schedule, and price. If you are using the waiver, bring questions about the service authorization to the follow-up conversation. Our Kentucky HCB Waiver guide explains the distinction.
For Louisville professionals and families coordinating from elsewhere
A referrer’s office and a participant’s home may be in different locations. The same is true when an adult child lives outside Kentucky and is helping someone in Louisville. Start with your own contact details and the care ZIP code; explain who should be involved in a private follow-up.
Professionals can use our home care referral page. Families can use the checklist for preparing for your first home care conversation. A requested schedule or start date still needs to be discussed with the team.

Talk with Better Life
You do not need to have every detail settled before you contact us. Share your own contact information and the ZIP code where help is needed. We can discuss the service, schedule, and payment questions that matter to your situation.
Please keep participant names, Medicaid IDs, diagnoses, and medical records out of the website inquiry. We will explain how to share necessary information privately.
