Referral Process

How to Refer

We work with all home care agencies and can send people where they desire to get services from. We welcome referrals to our office by fax, phone or in person from any physician, RN, APRN, Hospitals, Home Health or Mental Health Agency.

Required Information

• Client's name
• Social Security Number (SSN)
• Date of Birth (DOB)
• Address
• Medicaid number
• Doctor's name and number
